This Agreement is Made Between:

Mentor, {Sally Jane Douglas} and Client, {yourself}

Terms:

This agreement, will begin upon acceptance, and will include all agreed to sessions. Fees are due upon acceptance of this agreement. Full payment and direct debit payment plans are to be fulfilled as agreed.

The services to be provided by the mentor to the client are mentoring or tele-mentoring, as designed jointly with the client. Mentoring, which is not medical advice, therapy or counselling, may address specific personal challenges, successes, or general conditions in the client's life.

Confidentiality:

The mentor promises the client that all information provided to the mentor will be kept strictly confidential.

Mentoring Relationship:

Throughout the working relationship, the mentor will engage in direct and personal conversations. The client can count on the mentor to be honest and straightforward in asking questions and making requests. The client understands that the power of the mentoring relationship can be granted only by the client, and the client agrees to do just that: have the mentoring relationship be powerful. If the client believes the mentoring is not working as desired, the client will communicate that belief and take action to return the power to the mentoring relationship.

Declaration:

I understand that I am working with Sally Jane Douglas for mentoring in the realm of personal, interpersonal and emotional support at the agreed fee. This does not include medical support or technical support.

Mentoring is distinctly different from counselling, psychotherapy or psychoanalysis and does not deal with the diagnosis or treatment of emotional or physical problems. Since mentoring does not constitute medical consultation or treatment, health insurance does not apply.
